1. Diet: typically regular, avoid spices and alcohol for 1 week. Increase fluid intake to 3 quarts per day for the next 3 days. Anything in the fluid family counts except booze.
2. Activities: Bed rest for the next 24 hours. Remove penile wrap in 7 days. Some hydrogen peroxide impregnated into the dressing via a wash cloth for 5 to 10 minutes may help. Observe the head of the penis for any marked swelling, numbness or change in coloration (pale, blue, etc..). Bruising elsewhere is not necessarily of concern. If such occurs, call me first and you may be instructed to remove wrap immediately. Wear clean, loose, cotton underwear. Change underwear daily. If "blood spotting" occurs, apply Betadine ointment. May shower the day after surgery, if you wear a dry condom or cover the penis with "Saran Wrap" to protect the penis. Do not rub the suture area, but rather pat dry with a towel. No sexual penetration activity for at least 6 weeks or until incision completely healed and keratinized.
3. Medications: Keflex 250 mg. #28, 1 tablet 4 times a day for the next 7 days (if not allergic to penicillin). Tylenol (acetaminophen), 2 tablets as needed for moderate pain. Ultram 50 mg. every 6 hours as needed for severe pain.. Erections may be mollified with psuedoephredrine hydrochloride (formerly known as Sudafed, now a controlled item) taking 2, 30 mg tablets taken every 6 hours or before sleep as needed to control erections. Do not abuse, as Sudafed can have adverse effects on cardio-vascular function such as rapid pulse and high blood pressure.
4. Follow-Up: Call us tomorrow AM, let us know how you are feeling. Some spotting or drainage is to be expected. On the 10th day, take a warm tub bath for 15 to 20 minutes to soften sutures. Return to the office in 12 days for a look-see and suture removal, or visit your local doctor or emergency room. Needed are a bright overhead light, delicate plastic surgery scissors and pickups, and loupes. Please do not let anyone talk you out of suture removal by saying, "they are absorbable, they'll melt out." Yes, they ultimately will, but leave cross hatch marks.
5. You should not drive after surgery until the next day. We insist that you leave our facility either in a taxi, or vehicle driven by a friend/relative. You may feel capable of driving following discharge, but you are may be medically-legally under the influence of controlled substances if certain drugs have been administered, and your reflexes and judgement may be seriously impaired.
